Necessary Documents for Passport

Certificate of Citizenship in correct name OR the following documents if you do not have the C of C:

  • Foreign Birth Certificate
  • One parent's passport and BOTH parent's Identification (if BOTH parents do not go to apply for the passport, then the parent not in attendance must provide a notarized statement saying that he or she is allowing the other parent to apply for a passport AND a copy of that parent's identification. The statement and copy of ID can be on the same paper.)
  • Permanent Resident Card or Foreign Passport
  • Foreign Adoption Decree (will not accept the Readoption Certificate alone)
  • Passport Application
  • Two passport pictures
  • Payment in the form of check or money order
